A sharp photo or scan of the page with your photo and details. All four corners visible, no glare — ESTA reads the machine-readable zone from it.
Selfie photo
A recent, clear photo of your face, straight on, no hat or sunglasses. US authorities require this photo with every application filed on your behalf.
